dc.descriptionTítulo: Características Clínicas y Epidemiológicas de la mortalidad por Influenza humana tipo A H1N1 en el H.G.R. N° 1 del IMSS “Vicente Guerrero”, Acapulco, Guerrero. Objetivo general.- Describir las características clínicas y epidemiológicas de mortalidad por influenza A (H1N1) en el Hospital General Regional No. 1 “ Vicente Guerrero”. Material y métodos.- Se realizó un estudio de serie de casos, descriptivo y retrospectivo, para analizar las características clínicas y epidemiológicas de 8 defunciones por influenza tipo A H1N1, en el Hospital General Regional No. 1 del IMSS utilizándose expedientes clínicos, del 01 de Enero al 31 de diciembre del 2009. Se realizó un análisis descriptivo e inferencial en el programa SPSS17. Resultados. De las 8 defunciones por Influenza Humana tipo A H1n1 todos presentaron fiebre, astenia, adinamia, rinorrea hialina, tos, diaforesis y cianosis, siendo estos dos últimos los relacionados con mayor mortalidad. Las enfermedades preexistentes se presentaron en 5 pacientes (62.5%). La mayor proporción fueron hombres 6 defunciones (75%) y jóvenes adultos. De los 8 fallecidos 4 pacientes (50%) eran residentes del puerto de Acapulco. Se realizó radiografía de tórax obteniéndose en 7 pacientes (87%) infiltrados bilaterales. El promedio de estancia intrahospitalaria en este hospital fue de 10.5 días. La Biometría Hemática de ingreso fue normal en 4 pacientes (50%) . En 7 pacientes (87.5%) el inicio de la sintomatología y la primera valoración médica fue posterior a las 72 horas. En todos los pacientes el tiempo de inicio de los síntomas y el diagnóstico fue mayor a 72 horas. En 7 defunciones (87.5%) el tiempo transcurrido entre el internamiento y el diagnóstico fue mayor de 72 horas. La defunción se presentó 72 horas posteriores a su ingreso hospitalario en 7 pacientes (87.5%) . En todas las defunciones, el tratamiento fue inoportuno. Conclusiones. Las principales características clínicas de la mortalidad por influenza tipo A H1N1 fue la presentación en todos los casos de: fiebre, astenia, adinamia, rinorrea hialina, tos, diaforesis y cianosis. La comorbilidad se relacionó con enfermedades autoinmunes. En mayor proporción fueron afectados los pacientes del sexo masculino y jóvenes adultos menores de 40 años. Los pacientes fallecidos acudieron tardíamente a la atención médica una vez que presentaron los primeros síntomas, condicionando retraso en el diagnóstico y tratamiento oportuno.es
dc.description.abstractTitle: Clinical and epidemiological characteristics of mortality from human influenza A H1N1 in the HGR No. 1 IMSS "Vicente Guerrero", Acapulco, Guerrero. General Objective - To describe the clinical and epidemiological characteristics of mortality from influenza A (H1N1) Regional General Hospital No. 1 "Vicente Guerrero". Material and methods .- A study of case series, descriptive and retrospective study to analyze the clinical and epidemiological characteristics of 8 deaths from influenza A H1N1, Regional General Hospital No. 1 IMSS using medical records, 01 January to December 31, 2009. We performed a descriptive analysis and inferential SPSS17 in the program. Results. Of the 8 deaths from human influenza A H1N1 all had fever, fatigue, weakness, hyaline rhinorrhea, cough, diaphoresis, and cyanosis, with the latter two associated with increased mortality. Preexisting conditions were presented in 5 patients (62.5%). The highest proportion were men, 6 deaths (75%) and young adults. In relation to place of residence 4 patients (50%) were residents of the port of Acapulco. Chest radiography was performed in 7 patients were obtained (87%) bilateral infiltrates. The average hospital stay in this hospital was 10.5 days. The blood count was normal income in 4 patients (50%). In 7 patients (87.5%) the onset of symptoms and first medical evaluation was after 72 hours. In all patients the time of onset of symptoms and diagnosis was longer than 72 hours. In 7 deaths (87.5%) the time between the placement and diagnosis was greater than 72 hours. The death came 72 hours after hospital admission in 7 patients (87.5%) hospitals. In all the deaths, the treatment was inappropriate. Conclusions. The main clinical features of mortality from influenza A H1N1 was the presentation in all cases of fever, fatigue, weakness, hyaline rhinorrhea, cough, diaphoresis, and cianosis. La comorbidity was associated with autoimmune diseases. A greater proportion were affected male patients and young adults under 40 years.
