Descripción : El síndrome del túnel del carpo (STC) es la neuropatía periférica más común de la extremidad superior, la incidencia de STC es 99 por 100,000 persona-año, y la prevalencia es 3.4% en mujeres y 0.6% en hombres 1,2 , un poco más del 5% son personas que requieren el uso repetitivo de manos y muñecas en su actividad laboral Este síndrome provoca dolor, entumecimiento y adormecimiento en la palma de la mano y los dedos pulgar, índice, medio y a veces también el anular. Si la compresión sobre el nervio se mantiene en el tiempo y aumenta, las molestias irán progresando cada vez más hasta hacerse intensas, permanentes e invalidantes. La práctica de ciertas actividades como teclear, tejer, tocar violín, etc. determinaría un déficit de circulación en la zona del ligamento, el que iría endureciéndose y aumentando su grosor, así como continuar con procesos inflamatorios constantes y un aumento el área de fibrosis del túnel del carpo que va empeorando con el paso del tiempo y con la actividad ejercida. 6,7. Poco se conoce sobre su fisiopatología, se proponen tres teorías para explicarla:  La compresión, este aumento de presión causa obstrucción del flujo venoso, edema y por último isquemia del nervio.  La insuficiencia microvascular que conlleva a la hipoxia e isquemia del nervio.  La teoría vibratoria. Las cuales no se excluyen mutuamente y no se ha dilucidado completamente si la hipoxia se encuentre en relación directa o indirecta con el desarrollo de esta enfermedad y poco se sabe si la expresión del VEGF se encuentra regulada en este padecimiento como se ha comprobado en otros sistemas y órganos por la hipoxia. Algunos pacientes resuelven esta enfermedad de forma no invasiva con fisioterapia que consiste en sesiones de movilización del tejido conectivo, mejorando la circulación del ligamento y la zona de lesión, sin embargo el cuadro reincide y a menudo amerita tratamiento quirúrgico ,se han reportado que los resultados de este procedimiento varía de acuerdo a la sintomatología y la funcionalidad que presenta cada paciente que va del ( Ciénega y col.) del 74% al 6% , sin embargo la reincidencia es de un 60% de las parestesias y el dolor .Dentro del tratamiento farmacológico se encuentran los AINEs y el uso de No AINEs , sin embargo también existen el uso de otros fármacos como lo es el colágeno y polivinilpirrolidona ( Fibroquel ) el cual ha demostrado que modula los procesos inflamatorios crónicos se modifican a su vez algunos parámetros inflamatorios como IL-1, TNF-α , en las articulaciones y disminuye la actividad colagenolítica total a través de una baja significativa de las colagenasas independientes de calcio y disminuye la fibrosis y moléculas de adhesión como la molécula de adhesión intercelular 1 (ICAM-1) y VCAM-1 se observan disminuidas para los tejidos tratados con Fibroquel , con un cambio en el metabolismo celular , sin embargo no hay evidencia científica sustentable para el uso de este fármaco en el síndrome de túnel del carpo .
