Resumen : In order to evaluate the usefulness of Siriraj index for clinical differential diagnosis between supratentorial cerebral hemorrhage and infarction, we studied 38 patients with a diagnosis of cerebral vascular disease. The definitive diagnosis was confirmed by CT scanning. The aim is to apply the scale Siriraj Index as a clinical tool for differential diagnosis of cerebral vascular event agudo.La hypothesis is that the Siriraj Index is useful for the diagnosis of acute cerebral vascular disease. The variables were Siriraj Index, the result of the CT scan, headache, vomiting, diastolic blood pressure, markers of atheroma (diabetes mellitus, angina, intermittent claudication). Siriraj index was calculated with the formula (2.5 x level of consciousness) + (2 x vomiting) + (2 x headache) + (0.1 x diastolic blood pressure) - (3 x atheroma markers) - 12. +1 Values are considered indicative of hemorrhage, cerebral infarction -1, between 1 and -1 were doubtful diagnosis. We designed an observational, cross sectional and retrospective. The study population included patients admitted to the hospital emergency department and general area of the former Tlaxcala No.1 January 2010 to June 30, 2010, with acute cerebral vascular disease not related to trauma. Clinical scale was used and cited as Siriraj Index, using the measuring instrument will collect the criteria considered in Siriraj Index for variables to investigate the clinical record, the original notes and report emergencies axial tomography computed. Once the data collected will be applied statistical formula Siriraj Index (Annex 2) to obtain scores indicative of hemorrhage or cerebral ischemia. Statistical analysis was based on descriptive statistics and parametric tests as a percentage, mean and median, mode and range. Does not require informed consent because it is a minimal risk study, considering the Belmont Report. The results were handled with confidentiality. We found 27 patients with infarction and 11 with parenchymal supratentorial cerebral hemorrhage. Cerebral hemorrhage was more frequent in male gender while stroke was more frequent in female gender. The most significant variable for cerebral hemorrhage was severe, the alert status was more frequent drowsiness or stupor, with two cases of comatose paciertes; for myocardial significant variable was the headache, the alert status was more frequent drowsiness or stupor, without any case of coma. The diagnostic accuracy of the test was 77%.
Descripción : Con objeto de evaluar la utilidad del índice de Siriraj para establecer el diagnóstico diferencial clínico entre hemorragia e infarto cerebral supratentorial, se estudió a 38 pacientes con diagnóstico de enfermedad vascular cerebral. El diagnóstico definitivo fue corroborado por medio de tomografía computarizada de cráneo. El objetivo es aplicar la escala del Índice de Siriraj como instrumento clínico para el diagnóstico diferencial del evento vascular cerebral agudo. La hipótesis planteada es que el Índice de Siriraj es útil para el diagnóstico de enfermedad vascular cerebral aguda. Las variables a considerar fueron Índice de Siriraj , resultado de la tomografía axial computada, cefalea, vómito, presión arterial diastólica, marcadores de ateroma (diabetes mellitus, angina, claudicación intermitente). El índice de Siriraj fue calculado con la fórmula (2.5 x nivel de conciencia) + (2 x vómito) + (2 x cefalea) + (0.1 x tensión arterial diastólica) - (3 x marcadores de ateroma) - 12. Los valores +1 se consideraron indicativos de hemorragia, -1 de infarto cerebral, entre 1 y -1 resultaron dudosos para el diagnóstico. Se diseñó un estudio observacional, descriptivo, transversal y retrospectivo. La población en estudio incluyo a pacientes que ingresaron al servicio de urgencias del hospital general de zona No.1 Tlaxcala del primero de Enero del 2010 al 30 de Junio del 2010, con enfermedad vascular cerebral aguda no relacionada con el traumatismo. Se utilizó la escala clínica ya citada y mediante el instrumento de medición se recolectarán los criterios considerados en el Índice de Siriraj para las variables a investigar del expediente clínico, en las notas iniciales de urgencias y en el reporte de la tomografía axial computada. Una vez recabados los datos se aplicó la formula estadística del Índice de Siriraj (anexo 2) para obtener los resultados indicativos de hemorragia o de isquemia cerebral. El análisis estadístico se basó en estadística descriptiva y pruebas paramétricas como porcentaje, media y mediana, moda y rango. No se requiere de consentimiento informado dado que es un estudio de Riesgo Mínimo, considerando el Informe Belmont. Los resultados fueron manejados con confidencialidad. Se encontraron 27 pacientes con infarto y 11 con hemorragia cerebral parenquimatosa supratentorial. La hemorragia cerebral fue más frecuente en género masculino mientras que el infarto cerebral fue más frecuente en el género femenino. La variable más significativa para hemorragia cerebral fue la cefalea, el estado de alerta mas frecuente fue la somnolencia o estupor, con dos casos de pacientes en estado de coma; para el infarto la variable significativa fué la cefalea, el estado de alerta mas frecuente fue somnolencia o estupor, sin ningún caso de coma. La exactitud diagnóstica de la prueba fue de 77 %.
